Final view. This really isn't to hard to make just have to measurements and have an idea of how to fold and make the pockets...

I wanted to make this to work with a sheet of 12X12 cardstock to make the outside piece, so you could get two from one sheet. So this was my starting point.  Here is measurements below. Enjoy.

6 X 12  Cert. Celery score at 5 1/2" and 6 1/2"

Pockets you need to cut two.   4 3/4" X 7 7/8" score at 1/2" and 1" on three sides, then you will trim off the bottom outer three square of the pockets. Hope that's not to confusing.
